| Benedict House is a non-profit, supportive housing program for persons eighteen (18) years of age or older, living with AIDS.
The admission standards by which applicants will be assessed to determine eligibility for admission into our Congregate-Housing program are delineated below. These standards, must include, but are not limited to the following components and are subject to Benedict House administrative staff approval. |

Negotiable, based upon entitlement eligibility and ability to pay (includes utilities, case management, program participation, meals, laundry facilities and basic cable) |
| Requirements for Admission | ||||
| 1. | Diagnosis of AIDS, verified in writing by physician. | |||
| 2. | Homeless, or at-risk of homelessness. | |||
| 3. | Ability to perform activities of daily living including ambulating, eating, bathing, personal hygiene, toileting, medication administration, and dressing. | |||
| 4. | Ability to perform activities of daily living including ambulating, eating, bathing, personal hygiene, toileting, medication administration, and dressing. Assistance with medication through ELT or assistance with activities of daily living through the Cognitive Services Program is not necessarily a barrier to admission. | |||
| 5. | Referral will be made if a resident should become unable to perform these activities of daily living and requires a higher level of care facility as determined by their primary care providers. Community nursing visits will not create a barrier to service. | |||
| 6. | Participation in a treatment program is not a barrier to admission (i.e. methadone, substance abuse/mental health outpatient, etc.) but there must be a clear commitment to maintaining recovery and compliance with the mutually agreed service plan. | |||
| 7. | Director of Resident Services must verify applicant's mental capability to function safely within a Congregate-Housing program. | |||
| 8. | Physician must verify applicant's negative pulmonary TB status, in writing. | |||
| 9. | Applicant must review and agree to Congregate-Housing Lease, including service plan. | |||
| 10. | Program placement is at the discretion of Benedict House administration. | |||
Before Admission Applicant must complete admission intake procedures (including medical screen) and fully executed Occupancy Agreement, including service plan. The fact that applicant(s) may fully meet these Congregate-Housing criteria does not guarantee residency. |
